What health insurances does Purposed Physical Therapy accept? AKA what health insurances are we In-Network with?

  • All Blue Cross Blue Shield Plans (including out-of-state BCBS plans)
  • All United Health Care Plans (including out-of-state & subcontracted UHC plans)
  • Medicare
  • Most Medicare Replacement & Advantage Plans (Humana, Aetna, CDPHP, etc.)
  • Molina Marketplace
  • Veterans Affairs (VA)

*Your cost for Physical Therapy is determined by your health insurance plan, not us. When you call to schedule your first visit, 90% of the time we can tell you your cost based on your plan over the phone (by logging into the provider portal for your insurance).

*Most Medicare Replacement & Advantages plans we will have to call to determine your benefits, since most do not have a provider portal for us to access. Also because we are a private practice (where you always spend 60 minutes 1-on-1 with a doctor of Physical Therapy) your cost may be slightly increased in our office compared to the larger coporate settings (where you get less quality and rarely work with a doctor of PT).

   


   

What if Purposed Physical Therapy is Out-of-Network with my insurance?

  • Your per visit cost will be the same as our lower self-pay rate ($80).
  • You can still use your HSA card, if you have one.
  • Within 24 hours after each visit, we will email you a CMS-1500 (gold-standard of insurance invoicing) for you to submit towards your out-of-network benefits.

   


   

What if Purposed Physical Therapy is In-Network with my health insurance but I do not want to use my health insurance?

  • This may the case if you have a crazy high deductible, or your cost per visit is really high. Again, your cost is determined by your insurance company, not us.
  • Your per visit cost will be the same as our lower self-pay rate ($80).
  • You can still use your HSA card, if you have one.
  • If you opt to self-pay rather than use your in-network benefits, we legally will not be able to send you a CMS-1500 to self-submit towards your health insurance.

    


    

Does Purposed Physical Therapy treat patients with Tricare or Medicaid?

Yes, our Physical Therapist can treat Tricare and Medicaid beneficiaries, BUT we are not currently "an enrolled provider" with Tricare or Medicaid. Thus, legally we require to have these patients pay and be listed as self-pay ($80).

We have been trying to get in-network with Tricare for over 5 years now, and are unable due to Prisma Health controlling all the remaining Tricare contracts in our area. Purposed Physical Therapy does not believe in discrimination against our armed services, or those who require government assistance. If you have Medicaid, we legally cannot create an invoice for you to submit to your insurance. If you have Tricare we can create an invoice for you to submit for possible reimbursement!

If you would like Physical Therapy services to be covered by Tricare or Medicaid, and if you have a Physician referral for such, then we will be happy to recommend other providers in the area who are in-network and enrolled with Tricare and Medicaid. We always want what is best for our community, even if this means you need to be seen by another provider!

   


   

Does Purposed Physical Therapy work with Motor Vehicle cases?

Does Purposed Physical Therapy work with Workers Compensation or Personal Injury cases?

​Unfortunately, Purposed Physical Therapy does not work with Workers Compensation or Personal Injury cases.
